AUPE Local 002

For members with twenty years or more of membership, Local 002 offers a cash gift. For members with ten years of more of membership, Local 002 offers a plaque to the retiring member. It is important to note if there was a break in service because available records use the last commencement date as the beginning of service.

To be eligible, nominations must be received no earlier than three months prior to, but not later than three months after a member's retirement date. Plaques take approximately a month to produce, so applications should be submitted early enough to arrive on time if a reward ceremony is planned. Retiring members or friends can initiate the process by completing this form.

The information is requested to ensure that the nominee meets the criteria for the award and so that the gift can be sent at an appropriate time.

The following information is required:

• The retiree's name
• The retiree's department
• The retiree's address, and phone number
• The number of years of membership
• The email address of the person filling in the form

Optional information:

• Contact infomation for the nominator
• Is a retirement plaque being requested
• The dates of breaks in membership so total membership time can be calculated
• The date of a retirement function when the gift can be presented
• The name of a Local 002 member who would be appropriate to do the presentation
• The phone number of the presenter
